Harriet Hayes was a white female widow of Wm. Hayes. She was born July 22, 1940 in Elizabethtown, Ohio, and was a retired housewife at her death. Her father was Alex Guard of America, her mother was Eliza Miller of America. The informant for this certificate was Stella H. Collier of North Bend, Ohio (her daughter). Harriet died on April 6, 1921 at home, at the age of 80 years, 8 months and 14 days.
The certification of death, signed on April 7, 1921 by B. F. Lehmann, M. D. of Cincinnati, Ohio, was Uramia from chronic interstitial nephritis, duration 5 years, with a contributory cause of age. There was no operation before death, and no autopsy, results determined by physical examination.
Undertaker was E. J. Nerdhand. Burial to take place on April 10, 1921 at Maple Grove.
