I solemny swear that I was born at West Chester, in the State of Penna., on or about the 25th day of September, 1895, that my father J. Carroll Hayes was born in West Chester, Penna. ....., and that I am domiciled in the United States, my permanent residence being West Chester in the state of Pennsylvania, where I followed the occupation of Student. My last passport was obtained from Washington, and was marked "cancelled". I am about to go abroad temporarily; and I intend to return to the United States within 18 months with the purpose of residing and performing the duties of citizwenship therein; and I desire a passport for use in visiting the countries hereinafter named for the following puropose
France Relief Work for American Friends Service Committed Member of the Reconstruction Unit.
I intend to leave the United States from the port of New York City sailing on board the "Chicago" on August 25, 1917.

Decription of Applicant: 21 years old, forehead medium, mouth medium, chin square, 5 feet 9 1/2 inches, eyes Hazel, hair Brown, complexion dark, nose Straight, face Oblong.
Identification sworn to by Earnest L. Harvey, Assistant U S District Atorney, a native citizen of the United State residing at West Chester, Pa who swears he has know William Waldo Hayes for 18 years, know him to be a citizen of the United States and that the facts stated in his affidavit are true to the best of his knowlege and belief.
Applicant desire that the passport be sent to William Waldo Hayes at Haverford, College, Haverford, Montgomer Co., Pennsylvania.
There is a Form E Permit for Passport attached from Division No 4, Eastern District of Penn., Court House, Lancaster, Penn.certifying thet the application is mertorious as his both -
a. The applicant's order number is so far down the list that he cannot be included in the present draft.
b. The applicant has been granted exemption or discharge on Form 174 prepared by the Provost General Marshall for the reasons therein name.
There is a letter from the County of Chester Pennsylvania, Office of the Commissioners, dated 8/8/1917 to whom it may concern that "This will certify the William Waldo Hayes, aged twenty-one years registered with the Local Board No. 2 County of Chester, State of Pennsylvania, has appeared before the Local Boardfor Physical examination and has filed claim for discharge under Section 80 item I (Member of Religious Order of Friends) and is now proposing to enter construction work under American Friends Reconstruction Unit in France. We are satisfied the the application is meritorious and therefor discharge him under the above provisions." It is signed by David M. Golder, P, chairman and Jos. Southwood, Clerk.
The next page is a letter from American Friends Service Committee, 20 South Twelft Street, Phildelphia, PA dated 8//1917
"This letter is to introduce William Waldo Hayes who has been duly commission by this Committee to engage in reconstruction and relief work in France. He is a member of the Reconstruction Unit which this committee has been training at Haverfore, Pa, with the approval and under the direction of the American Red Cross War Council. He is a member of a party of fourteen whom we are sending in advance of the Unit at the request by cable and letter of J. Henry Scattergood and Morris E. Leeds, who form our commission in France, and who are also members of the Red Cross Commission.
William Waldo Hayes will first enter the work carried on under permit from the French Government by the War Victims Relief Committee of the English Society of Friends with offices at 91 Bishopsgate, London, E/ C/ and at Hotel Brittanique, 20 Avenue Victoria, Paris. He will retain his character as as American Red Cross worker, and will later join the independent work of this Committee carried on under the American Red Cross.
William Waldo Hayes is an American citizen of American parents. We shall appreciate every assistance that is extended to him in carrying out this work."

Attached is a letter dated 1/6/1919 from the American Red Cross stating:
"Sir, This will serve as an endorsement by the American Red Cross of the passport application of William Waldo Hayes who is going to Palestine to engage in Red Cross work under our Commissioner General for Europe. Mr. Hayes will be obliged to travel in Great Britian, France, Italy and Palestineand we should greatly appreciate the inclusion of the above mentioned countries in the passport.
There is also a letter from the War Department, Office of the Chief of State, Washington, date 12/31/1918 stating -
the War Department has no objection to Hayes, William Waldo of 436 North Church Street being sent for duty with the A. H. F., in connection with American Red Cross. signed by J. S. Moore, Captain U S Army for M. Churchill, Brigadier General, General Staff, Director of Military Intelligence.
Also attached is an amendment to the passport requesting to travel to Greece and proceeding to France as per a letter from the Palestine American Red Cross ordering him to report to Paris. The letter mention applicants occupation is Chemist, A. R. C.
