Harris, a native of this city residing #2027 Constance St, who hereby declares that on the Fourth day of November 1896 (Nov 4 /96) at his residence was born a female child, named Hazel Adelaide Harris (white) Lawful issue of deponent a native of this city aged 39 years occupation Cotton Inspector and Emerita Davis a native of this city, aged 38 years.
