Be it remembered, That on this day to-wit: the Eleventh of May in the year of our Lord One Thousand Eight Hundred and Ninety Six and the One Hundred and one of the independence of the United States of America, before me, S. R. Olliphant, M. D., President Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages, in and for the Parish and City of New Orleans, personally appeared Charles F. Walter, native of this city residing at No 2139 S Liberty St who hereby declares, that on the thirteenth day of last April (10 Apr 1896) at his residence was born a female child, named Henrietta Madeline Walter (white) lawful issue of Deponent a native of this city aged 28 years, occupation laborer and Pauline Asmus a native of this city, aged 20 years.
Thus done at New Orleans, in the presence of the aforesaid C F Walter as also in that of Messrs. [two illegible signatures, looking mostly like P H L and L B Hellbach] both of this City witnesses, by me requested so to be, who have hereto set their hands together with me, after due reading hereof, the day, month and year first above written.

Their daughter, 8 year old Audrey Harris, was born in Louisiana, father in Virginia, mother in Louisiana. 63 year old widower, Charles Walters is Etta's father. Etta's sister, Alva Walters, is 26 and single.

Medical:
In the last year of her life, she became confused at times. She once called me to her house because her kitchen table was covered in ants. When I got there, not a one was to be found. I was very concerned about her living alone, as she was cold even in hot New Orleans summers, and would light gas space heater at night.

She became physically ill (internal bleeding), but various tests were not conclusive. She was hospitalized in Touro. She did not want any operations, but the surgeon treating her said she may have a simple blockage. I convinced her to undergo exploratory surgery. She was found to have cancer, which was removed. She stayed in the hospital for several weeks, until she was told her level of care no longer justified it. We tried her living at home with sitters, but eventually put her in skilled-care nursing at Jo Ellen Smith nursing home. I visited her daily, but she was almost deaf , vision very poor, mind unfocused, and could barely talk at all. After a few weeks her condition worsened, and she was moved next door to Jo Ellen Smith Hospital, where she died a few days later.
